Permits:

Annual body art establishment permit fee: $633.00

Temporary event permit: $264.00

Certifications:

Initial annual body art operator license fee: $528.00

Renewal annual body art operator license fee: $106.00

Apprenticeship fee: $106.00

Temporary tattoo artist fee: $53.00

Testing Fees

Initial testing fee: $106.00, which covers two attempts to pass the examination, and which amount will be credited toward the initial annual body art operator license fee.

Additional attempts to pass the examination: $53.00, which will not be credited toward any license or permit fees.

Late Fees:

Late Fees: $27.00 for every month expired
