  • Annual body art establishment permit fee: $618.00
  • Temporary event permit: $258.00
  • Late Fees: $26.00 for every month expired

Certifications:

  • Initial annual body art operator license fee: $515.00
  • Renewal annual body art operator license fee: $103.00
  • Apprenticeship fee: $103.00
  • Temporary tattoo artist fee: $52.00
  • Late Fees: $26.00 for every month expired

Testing Fees

  • Initial testing fee: $103.00, which covers two attempts to pass the examination, and which amount will be credited toward the initial annual body art operator license fee.
  • Additional attempts to pass the examination: $52.00, which will not be credited toward any license or permit fees.
