This form is used to amend a death certificate when there has been a spelling error, when items have been left blank, or if items need to be changed.
In most cases items and be changed or fix by the informant of the death certificate, the funeral director, or a family member of the deceased. Some cases, additional supportive documents must be presented along with this application. Other cases, like medical information can only be changed by the medical attendant or coroner that certified the death. The form has a detailed list of whom can request changes for each item.
There is a filing fee of $15, check or money order is accepted. Payable Vital Statistics Unit DHSH. For an additional $20, a corrected certified death certificate can be mailed to you.
